
Patient Navigator at Aga Khan Education Service, Kenya (AKESK)

- Reporting to the Project Manager, the successful candidate will engage with newly diagnosed patients from Homabay County primary health centers and surrounding communities to receive timely, patient-centered care at the Homabay County Referral Hospital (HCRH), Aga Khan Hospital Kisumu(AKHK) and other Hospitals.
- Engage patients, create a trusting relationship, navigate patients to HCRH or AKHK hospital appointments, and communicate with a patient's entire care team based on clearly outlined goals and identified patient needs.
- Assist patients in the coordination and completion of appointments inside the AKHK/HCRH Cancer Center and help decrease barriers to timely cancer care and treatment.
- Initiating contact with and responding to requests from patients, other team members and associated healthcare agencies and providers
- Assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way
- Provide patients, relatives and other services with correct information
- Accompany patients to specialty and imaging centers when needed to provide support and advocacy.
- Maintain regular communication with the patient's providers through clinical messages via emails, phone calls and case review meetings.
- Talking to patients over the telephone and ensuring that their queries are dealt with efficiently
- Communicate with patients regarding appointments and admissions at the request of the medical staff and as per their TCA schedule.
- Working within clinical team to provide a comprehensive Referral Service
- Managing administration of referral spreadsheets
- Produce monthly reports on program activities compiling data from databases and writing up case examples.
- Be involved in evaluation and research activities in the program as needed.
- Diploma or equivalent in psychology or social work.
- Two to three years of experience working in community or health settings preferred in a similar or related role.
- Demonstrated commitment to impacting the care of high-risk patients.
- Knowledge of complex patient issues preferred.
- Experience working as a patient navigator/community health worker preferred.
